Description des services: Provides assessment, diagnosis and treatment planning services for mental health-related illnesses for adults aged 18 to 64. The assessment process may include a screening phone interview with a mental health clinician, followed by an assessment which is generally done by a psychiatric resident and psychiatrist together. Program works with the individual to decide on a treatment care plan. The treatment care plan may include medication options, psycho-social interventions, individual or group therapies, or recommendations for educational tools and self-help resources. Depending on the care needs, the plan may include time limited follow-up visits in the clinic or direction to appropriate resources outside of Sinai Health. The plan will be communicated to the referring physician.


Procédé et formulaires: Physician referrals are required. To make a referral, please call or fax the referral form that is found on the website. Participants must be 18 years and over with a variety of psychiatric/emotional problems. Participants must live in the city of Toronto.
