Description des services: Outpatient, community-based program for individuals 14-35 years old experiencing early stages of psychosis * clinical partnership between Halton Healthcare, Joseph Brant Hospital, North Halton Mental Health Clinic, Halton ADAPT and the Schizophrenia Society of Ontario * this recovery focused program helps individuals achieve their goals at work, at school and within their social networks, thereby reducing the impact psychosis can have on one’s life * team includes psychiatrists, nursing, social work, occupational therapists, peer support and addiction counsellors


Services include:
  • psychiatric assessment and treatment
  • medication and symptom management
  • case management, group and individual counselling
  • family education and support
  • relapse prevention planning and strategies
  • addictions counselling
  • peer support
  • community program referral and illness management instruction after discharge
  • assistance with education, work, and housing
