Description des services: Provides a comprehensive range of outpatient services to children, youth, adults, and seniors * referrals are screened and processed by one-Link

Outpatient services for adults:
  • Adult Eating Disorders Program (multidisciplinary assessment to determine eligibility and short-term treatment for adults with mild to moderate eating disorder symptoms)
  • Community Case Management (community-based case management services for individuals aged 18 years and over living in the Halton community with serious and persistent mental illness)
  • Mental Health Day Program (six week group-based program meets daily and provides group and individual-based care with a focus on crisis management, symptom stabilization and skills development)
  • Medication Clinic (offers medium-term, hospital level medication management for adults diagnosed with serious and persistent psychiatric illnesses)
  • The Integrated Addiction Medicine Clinic (IAMC) provides individuals with rapid access to time-limited addiction care from any Halton Healthcare service including our three (3) Emergency Departments and all inpatient units * the clinic works in close partnership with Halton ADAPT
  • Psychiatric Consultation (consultation, assessment and diagnosis by a psychiatrist for adults 18 years and over)
  • Psychotherapy Services (individual and group therapy for individuals experiencing depression and anxiety disorders)
  • The Phoenix Early Intervention in Psychosis Program (clinical partnership between Joseph Brant Hospital, Halton Healthcare, North Halton Mental Health Clinic, Halton ADAPT and the Schizophrenia Society of Ontario that provides comprehensive clinical services for individuals in their first episode of psychosis)
  • Urgent Care Clinic (UCC) is a time-limited service for individuals experiencing urgent mental health concerns who require crisis stabilization * the team is staffed by psychiatrists and nurses
  • Crisis Outreach and Support Team (COAST) 1-877-825-9011
  • Older Adult Mental Health Clinic provides assessment, consultation, treatment and follow-up services to older persons with mental health issues, including behavioural and psychological complications associated with neurodegenerative diseases
  • Halton Geriatric Mental Health Outreach Program 905-681-8233

Outpatient and community services for children and adolescents
  • Child and Adolescent Clinic (provides short-term outpatient treatment for children and youth under the age of 18 years who are experiencing moderate to severe symptoms of mental illness)
  • Child and Adolescent Eating Disorders Clinic (provides multidisciplinary short-term treatment to children and adolescents under the age of 18 with moderate to severe symptoms * this includes individuals who have recently been discharged from hospital for medical stabilization, or who may experience acute medical crisis if appropriate intervention is not offered)
  • Navigator Program (provides short term support and case management to children, youth and their families under the age of 18 years who are at risk for hospitalization or who are transitioning from the inpatient setting or the Emergency Department)
  • Psychiatric Consultation (offers consultation, assessment and diagnosis by a psychiatrist for adults 18 years and over)
  • REACH Day Program (six-week intensive outpatient program provided for children and youth aged 12-18 * an alternative to inpatient hospitalization and follow-up from an inpatient admission)
  • Reach Out Centre for Kids (ROCK): assessment and treatment of children, adolescents and families * mobile crisis line 905-878-9785
  • Distress Centre Halton: support people in the community to better cope with crisis, loneliness, and emotional stress * 905-849-4541 (Oakville) * 905-877-1211 (Halton Hills/Milton) * 905-681-1488 (Burlington)
