Services include:
Primary care
- services are provided by a team of family physicians, nurse practitioners, nurses and other medical professionals
- services including care for children, prenatal care, minor procedures, gynecological care, adult health maintenance, cancer screening, and chronic disease management such as diabetes, high blood pressure
- OHIP or IFHP (Interim Federal Health Program) is not required in order to access primary care services
Diabetes Education- program is designed to improve the quality of life of people aged 18 and older living with prediabetes, Type 2 diabetes, and those at risk of developing Type 2 diabetes
- services are provided by a Registered Nurse and Registered Dietitian, through one-on-one counselling and group programs
Health Care Navigation and Support- focus on preventative care for all ages
- proactive support to improve access to healthcare and wraparound social services
- one-to-one and group programming are available
