Description des services: Provides supportive housing and outreach case management services throughout Halton for people diagnosed with a serious mental illness * services are based on a flexible, rehabilitative model of support and are tailored to meet the varied needs of the people * volunteering opportunities available


Programs include:
  • Housing and Homelessness
- Residential: four residential settings with dedicated staff support provide higher supportive housing options for individuals who would like to build on the skills necessary for independent living
- Supported housing: Rent-geared-to-income apartments based in Halton with case management support
- Housing First: the Housing First Program centers on first moving people experiencing homelessness into independent and permanent housing and then providing additional supports and services such as physical and mental health, education, employment, substance use and community connections
- HOMES Program: a community partnership initiative that provides supportive housing for adults who have been diagnosed with a serious mental illness who are homeless or at risk of becoming homeless * this program provides rent supplement housing with dedicated supports throughout the Region of Halton
  • Assertive Community Treatment Team (ACTT): for clients with severe and persistent mental illness, with priority given to people with schizophrenia, other psychotic disorders and with bipolar disorder
  • Outreach Case Management Program: provides supportive and flexible outreach support services to individuals 16 years of age or older with a diagnosed mental illness living in the community with the goal of promoting wellness, independent living and reducing the frequency and duration of hospitalizations
  • Core Supportive Housing Program: provides safe, secure, affordable, and decent accommodation in a variety of settings * rent geared-to-income housing
  • Justice Supportive Housing Program: provides affordable accommodation and flexible supports for people with serious mental illness who have come into conflict with the criminal justice system
  • Peer support: ACTT Peer Specialist who provides peer support to individuals who are receiving services from the Assertive Community Treatment Team * HOMES Peer Specialist who provides peer support to individuals receiving services from the HOMES Program
  • Nourish and Flourish Program: offers a holistic approach to enhancing food security and access, nutrition information, life skill building, social inclusion, food literacy, improved wellness and enhancing physical and mental health in the community * contact info@summit-housing.ca for more information
  • Anti-Human Trafficking: housing agency for the Anti-Human Trafficking Initiative in Halton * based on a Client Centered Support Model, support, advocacy and a range of housing is available to survivors of human trafficking * for referrals and more information about this program, contact SAVIS of Halton
